Kitchen Craft Dealer Access Request

If you are currently a dealer and would like to access Kitchen Craft's Dealer Web,
Please fill out the request form below.

The minimum requirements for using KC's Dealer Web are:

Microsoft Internet Explorer 5.5SP1 or greater
A video resolution of 800x600 with a palette of 65536 colors or greater.

Account Number: *
	(Your Kitchen Craft account number. This is what you will use to log in.)
Dealer Name: *
	(Name of your company.)
Administrative Contact: *
	(This should be the name of the person who will be responsible for control over your usage of the KC Dealers Web.)
Contact Phone Number: *
	(The Administrative Contact's phone number.)
Email Address: *
	(The Administrative Contact's Email address.)
User Password: *
	(Desired password for normal use. When you log on with ths password you will not be able to change any account information.)
Administrative Password: *
	(Desired password for maintaining account information.)
Additional Accounts:
	(If you have additional accounts whose orders you would like to be able to see under this account, enter their Kitchen Craft account number(s) here. Note: You will still use your account number you specified above as your log on.)
